From 1 July 2022, all Victorian Primary Care Partnership program funding transition to the Local Public Health Units. Primary Care Partnership functions for the South East Melbourne region is now being delivered through the South East Public Health Unit (SEPHU).

The transition of Primary Care Partnerships funding into Public Health Units aims to build on this success and will provide a platform to deliver a more coordinated and integrated approach to prevention and health promotion across Public Health Unit catchments. This presents SEPHU with a unique opportunity to plan and deliver prevention activity at scale across the South East Melbourne corridor. 

Engagement with stakeholders and partners is crucial to the success of this transition and will be a key focus as SEPHU develop their prevention strategy into the future.
