Primary Care Partnerships transitioned to Local Public Health Units

From 1 July 2022, all Victorian Primary Care Partnership program funding transition to the Local Public Health Units. Primary Care Partnership functions for the South East Melbourne region is now being delivered through the South East Public Health Unit (SEPHU).

PCP Transition Announcement

We're pleased to announce that we have received advice from the Secretary of the Department of Health, Prof. Euan Wallace AM that all PCPs will be transitioned into Local Public Health Units (LPHUs).

Professor Wallace noted that the establishment of LPHUs in a renewed and contemporary public health system offers a unique opportunity to improve population health outcomes for local communities by consolidating skills, knowledge and governance across a catchment area. It offers a consistent authorising environment to influence system level change and a renewed focus on place-based health promotion and prevention.

For Southern Melbourne PCP, this transition will be into the South East Public Health Unit.

PCP program transition begins

We're excited to share that the Department of Health has confirmed that the current arrangements for the PCP program will continue until March 2022!

Over the coming months the department will work together with PCPs to transition to a new governance model as part of a wider system reform which takes into consideration Public Health Units, Clusters and Community Health.

The Victorian Government is yet to confirm funding for PCPs beyond December 2019. Can we afford to lose this valuable platform?

For almost 20 years PCPs have brought together local organisations to achieve better health and social outcomes for their communities. The 28 PCPs across Victoria connect more than 850 organisations from health, education, community services and local government with their local communities.

Over 40 organisations connected through Southern Melbourne Primary Care Partnership in just the last 12 months to address family violence, elder abuse, indigenous access, health literacy, healthy eating, consumer engagement, service integration and more.

Elder Abuse Prevention Literature Review Launch

The SMPCP Elder Abuse Prevention Network were thrilled to have Commissioner for Senior Victorians and Ambassador for Elder Abuse Prevention, Gerard Mansour officially launch Preventing Elder Abuse: a literature review for the SMPCP Elder Abuse Prevention Network today. The document is a rapid review of primary prevention literature as it relates to elder abuse.
